Personal Supports Services

 

In-Home Support Services provide 4-24 hour assistance for activities of daily living and other duties necessary to maintain the individual in supported living. The support worker provides companionship and personal care and may assist with or perform activities of daily.

Life Skills Development 1- Life Skills Development Level 1 was formerly known as companion services. It includes nonmedical care and socialization activities provided to an adult on a one-to-one basis or in groups of up to three recipients

 

Life Skills Development Level 3, formerly known as adult day training, supports the individual to participate in valued experiences in the community, including volunteering, job exploration, accessing community resources, and self-advocacy in work-like settings that are age and culturally appropriate.

 

Respite Care

Description:  Respite care is a service that provides supportive care and supervision to a recipient when the primary caregiver is unable to perform these duties due to a planned brief absence, an emergency absence or when the caregiver is available, but temporarily physically unable to care for or supervise the recipient for a brief period.Place of Service:  This service may be provided in the recipient’s own home, family home or foster home. The recipient may also go to a licensed group, foster home, or assisted living facility to receive the service. Overnight respite care can be provided only in the recipient’s own home, family home, licensed foster home, group home or ALF. This service cannot be provided in the provider’s home.

 

 

Transportation Services provide rides to and from the individual's home and community-based waiver funded services.
